Itching, unusual discharge or a smell? Treating gynecological infections
Infections are one of the commonest reasons women come in. They may be bacterial, viral, fungal or protozoal. Left untreated they lead to chronic inflammation, fertility problems and complications in pregnancy.
Testing here is thorough and treatment is chosen for your case. PCR, culture and microscopy are used to identify exactly what is causing the problem.
Thrush treatment: when it is thrush and when it is not
Vaginal thrush (candidiasis) gives itching, burning and a thick white discharge without a strong smell. It is treated with an antifungal vaginal suppository or cream, or a single tablet of fluconazole, and usually settles within a few days. In pregnancy only local treatment is used.
Many women who treat “thrush” themselves have something else: bacterial vaginosis (thin grey discharge with a fishy smell), trichomoniasis or irritation from washes and pads. Antifungals do nothing for those, so if a pharmacy course has not helped, or it is back within two months, come for a swab rather than buying another pack. I take it at the same visit.
Oral thrush — white patches in the mouth — is not a gynecological problem: it is treated by a GP or a dentist. On this page I mean vaginal and vulvar thrush.
What is treated
Sexually transmitted infections
Chlamydia, gonorrhoea, trichomoniasis, Mycoplasma genitalium, genital herpes, HPV.
Candidiasis (thrush)
A yeast infection caused by Candida, common after antibiotics or when immunity is low.
Bacterial vaginosis
An imbalance of the vaginal flora, with opportunistic bacteria taking over.
Inflammatory conditions
Vaginitis, cervicitis, endometritis, adnexitis — inflammation in different parts of the reproductive tract.

Important
Many infections cause no symptoms at all, or almost none. Testing is worth doing when you change partner, when you are planning a pregnancy, and whenever you are unsure.

Results
PCR takes up to 10 days, culture 5–7. The results are read together and the organism identified.
Prescribing
A regimen chosen for the organism, where it is, and what it is sensitive to.
The course
Take the medication exactly as prescribed. Where needed your partner is treated too, so the infection does not come straight back.
Tests of cure
For some infections — gonorrhoea, chlamydia in pregnancy, Mycoplasma genitalium — a repeat test a few weeks after the course confirms it has cleared.
