The two approaches
There are two main ways of scanning in gynecology. Which one is used depends on what needs to be seen and on the individual patient.
Transvaginal ultrasound
The probe is placed in the vagina. This gives the clearest view of the uterus, ovaries and endometrium, showing detail an abdominal scan cannot. There is no need to fill your bladder.
Transabdominal ultrasound
The probe is moved over the abdomen. Used in pregnancy after the first trimester, for a general overview, and for women who have not had intercourse. A full bladder gives a better picture.
When an ultrasound is indicated
What the scan shows
The uterus
Size, position and shape. The endometrium — its thickness and structure. Fibroids, polyps, adenomyosis and developmental anomalies.
The ovaries
Size, structure and the number of antral follicles. Cysts, both functional and endometriotic, tumours and polycystic changes.
How to prepare
Transvaginal ultrasound
No preparation needed. Your bladder should be empty — just use the toilet beforehand.
Transabdominal ultrasound
Fill your bladder: drink 0.5–1 litre of water an hour before, and do not empty it until the scan is over.
The best day of the cycle
A routine scan: days 5–7, just after your period ends, are best for assessing the uterus and ovaries.
Folliculometry: several scans across one cycle.
With acute symptoms: any day.
